New accounts wishing to place an order must place their initial order by phone after this new account application has been completed.  Fax a copy of the prescribing physician's current DEA license and this completed form to:

Shipping Information
Orders over $100 receive free ground shipping.  Orders that are under $100, requested overnight shipment, or require overnight shipment will be charged shipping fees.

In order to sustain strict USP compliance, all steroid injections require a two week quarintine period to validate sterility.
