New accounts wishing to place an order must complete a New Account Application and return it to Cantrell Drug Company either by facsimile or mail:
Upon application approval, you will receive a Compounding Contract. Please return the signed contract, along with a copy of your current DEA license, to Cantrell Drug Company by one of the following methods:
Initial orders should be placed via telephone (Toll Free 877-666-5222 ext. 109). Subsequent orders may be placed via phone, fax, or e-mail.
